Taibu Community Health Centre serving the GTA’s Black-identifying community, provides comprehensive primary care services in a culturally affirming environment. Their holistic approach includes diagnosis, treatment, and management of acute and chronic illness, with a focus on addressing health disparities within the Black community.
Services Provided:
Primary Care programs, Chronic Disease Management Programs, Francophone services, Online appointment bookings, and Indigenous services. Community Programs for Families, Pregnant Women, Seniors, Black Men, Youth, and The Aya Circle of care; HIV case management. *Toronto Community Crisis Service, and Drop-in Case Management. Youth Programs Offered:
A Leadership program for youth aged 15-24 that address the impact of mental health challenges faced by Black youth.
Youth have access to an outreach worker for support and counselling.
Address the barriers to graduation and employment experienced by Black youth.
An intensive support program for students having challenges within the educational system.
A wrap-around program for African Canadian and Caribbean Black youth and their families facing mental health and substance abuse issues.
